Capsular Contracture: A layer of scar tissue known as a capsule CONTRACTURE: invariably forms around all breast implants. The bag of scar surrounding an implant usually remains soft and pliable and has little effect on the contour or texture of the breast.

In a significant percent of patients, the scar tissue membrane can undergo progressive thickening and shrinkage, a phenomenon known as capsular contracture, or encapsulation. There are numerous Grades of scar membranes, ranging from mild to severe. The scar contracture can be graded from I to IV.

GRADE I - Soft breasts and easily compressible.

GRADE II - Slightly firmer than desired, but no visible deformity.

GRADE III - Firm with external deformity notable. Breast assumes a spherical appearance.

GRADE IV - Extreme firmness, marked deformity, and hardness, associated with pain and tenderness. Treatment generally requires removing the scar tissue around the implant, replacing the implant and changing the position of the implant when appropriate.
